National Alzheimer's Buddies (NAB) agree to provide you with the use of tablet for work purposes only. They will pay for the tablet and protective case. By signing this contract you are agreeing to (please check each box): *

1. Take good care of this equipment.

2. Report any damage, loss, theft or malfunction to NAB immediately.

3. Pay for any repairs or replacement due to accidental damage, loss or theft.

4. Pay for any charges incurred for use of the tablet outside, contract limit.

5. Return it in good condition to the office should you cease to be a partnered facility with NAB.

6. After visit with Buddies, the tablet will be stored in a safe location.

7. Tablet will be prioritized for NAB use.
